NEW PROTOCOLS AND STRATEGIES FOR DIRECT ESTHETIC RESTORATIONS IN THE SMILE FRAME
Lesson 1. Indications & Limits of Direct Bonding in Anterior Esthetics
A key element in successfully using Direct Bonding in the smile frame is the deep understanding of its indications and limits. Direct restoration techniques can be used not only for cavities (Class III to V carious or non-carious lesions), but also in case of post-orthodontic esthetic corrections, form and color enhancement, veneers and also in combination with prosthetic restorations to enhance the overall treatment esthetic outcome. Direct Bonding is - and will remain - the gold standard and the preferred treatment option in the aforementioned scenarios, due to its simplicity, effectiveness and esthetic reliability. Resin Bonded Composite (RBSs) is not surpassed by any forthcoming technology in terms of reliability, esthetics and cost-effectiveness.

Lesson 2. The Natural Layering Concept: Latest Developments & Clinical Application
Color and anatomical integration of direct bonding restorations is paramount to meet the esthetic needs and expectations of our patients. Complex Layering multiplies the risk of error proportionally to the number of layers, and complicates the shade selection process. On the opposite, the Natural Layering Concept - based on the optical characteristics of the natural element - enables the Professional to perfectly match tooth shade through a massively simplified approach, increasing reliability and success of direct bonding procedures.

Lesson 3. Treatment Planning & Diagnostic Tools for Advanced Direct Bonding: 1D / 2D / 3D
A key to free-hand bonding in esthetic therapy, as for any other restorative strategy, is appropriate treatment planning, that must observe at the same time precise detail and proportion criteria. Depending on the extent of form or dimension/proportion enhancement, analog or digital tools allow the clinician to create a detailed esthetic project. Available methods include, either individually or in combination, the use of calipers, 2D imaging with lines drawn over the smile though Power Point or keynote, the use of digital templates, and finally 3D imaging. In addition, a waxup / mockup can be used to foresee extra- or intra-orally the new smile configuration.

Lesson 4. Restorative Protocol: from Layering to Polishing
Last but not least, free-hand bonding success relies on precise clinical restorative protocol selection. The first instrumental step is the build-up and layering of dentin and enamel shades control, a set of procedures largely simplified by the adoption of a bi-laminar approach (Natural Layering Concept), while an index may be needed in case of large corrections or restorations. Finishing and polishing procedures represent the next crucial step, performed in 4 steps and following 3D visual control. This lecture shows a simple method that allows to obtain the desired level of gloss of tooth-coloured restorative materials without losing the details of the restoration surface micro-anatomy.

INTERCEPTIVE & SELECTIVE TREATMENT OF TOOTH WEAR: A BREAKTHROUGH IN MANAGING EROSION AND ATTRITION PATHOLOGIES
Lesson 1. Diagnosis, Risk factors & Modern Treatment Strategies
The treatment of tooth wear requires different strategies.The criteria at the basis of the different restorative strategy are: nature of the defect (erosion, abrasion and attrition or a combination of these risk factors), Severity patient’s age Compliance socio-economic conditions. The 3 basic strategies are “preventive”, “interceptive” and “restorative”. While preventive measures are part of any tooth wear treatment strategy, the main difference between interceptive and restorative approaches is the extension of lesions to be treated and restorations to be performed. Interceptive strategy is first to be installed for limited and moderate tissue loss, using a no-prep approach and direct composite application (through either free-hand or molding techniques). More severe cases require an indirect/CAD-CAM approach, and mandatory tooth preparation, making it significantly more invasive, technically challenging and expensive.

Lesson 2. Direct-Interceptive Treatment Strategy: Free-Hand Bonding Protocols
While no formal consensus has been reached upon a rigid tooth wear treatment protocol, this lecture aims to describe the basic steps for treating limited to moderate tooth wear, thanks to a minimally invasive approach that involves the use of composite and allows ot avoid tooth preparation.
The process starts with a proper clinical diagnosis, followed by esthetic and functional planning on models, with either analog or digital wax-up. This allows to determine precisely the needed change (increase) of the VDO (Vertical Dimension of Occlusion), to create space for restorative material, to improve esthetics and function. Information can be transferred to the mouth using direct techniques and fully no-prep approaches (using only sand-blasting) to condition tissues or restorations to be modified.

Lesson 3. Direct & Indirect Treatment Strategies: Molding & Hybrid clinical protocols
In moderate to severe tooth wear cases, the simple and effective free-hand approach is indicated, while full molding techniques can be seen as an alternative method. In case of more extensive tooth wear or in presence of severe bruxism, direct composites can reach their structural limits, and stronger materials must be taken into consideration, especially in the molar areas. A wise treatment strategy sees a combination of direct and indirect restorations (selective treatment strategy) to exploit the benefits of different materials and techniques. This philosophy of work, that seeks to adapt and individualize treatment according to the tooth wear level and biomechanical features of different zones, represents a true game-changer in tooth wear management.

Bio-Esthetic Adhesive Dentistry
Dr. Didier Dietschi was licensed in 1984 and got his doctoral and Privat Docent degrees in 1988 and 2004, respectively, at the University of Geneva, Switzerland. He also got a PhD degree in 2003 at the University of ACTA, Netherlands. Following a 6-year period of full time teaching and research activity in Operative Dentistry and Periodontology, he started a part time activity in a private office in Geneva, dedicated to aesthetic restorative dentistry. He holds now positions of adjunct Professor at CASE Western University (USA) and senior lecturer at the University of Geneva. Dr. Dietschi has published over 115 clinical and scientific papers and book chapters on adhesive and aesthetic restorations; he also co-authored the book “Adhesive Metal-free Restorations”, edited in 1997 by Quintessence and translated in 7 languages. Dr. Dietschi is internationally acclaimed for his theoretical and practical teaching programs on adhesive & aesthetic restorations
